Form 8A: Application (Divorce)
This starts the case. It records both spouses’ details, the marriage, the separation, residence in Ontario, children, and what you’re asking the court for. In an uncontested divorce, you’re usually asking only for a divorce (and sometimes costs). The same form works for joint and sole applications; you indicate which one you’re filing.
Form 6B: Affidavit of Service (sole applications)
In a sole application, a process server personally serves your spouse and then swears Form 6B to prove it. Joint applications skip this step entirely.
Form 36: Affidavit for Divorce
This is your sworn evidence. It confirms the facts in the application, your separation, that there’s no reasonable chance of reconciliation, and, if you have children, the child support arrangements. It must be sworn or affirmed before a commissioner for taking affidavits. The judge relies on it to grant the divorce without a hearing.
Form 25A: Divorce Order
The draft order the judge signs. The court usually asks for copies and stamped, addressed envelopes so the signed order can be mailed to both spouses.

- Marriage certificate (or certified copy of the registration of marriage). If it isn’t in English or French, include a certified translation.
- Child support information if you have children, such as income details and any separation agreement.
- Any existing court orders about the children or support.
